The form can be tricky so here are a few tips that worked for me. It did not work for me in Chrome and only worked for me in IE. Also, if you are viewing this from a mobile device it will not work. Also, make sure you click on Find a Center at the bottom of the form and choose your closest location before hitting submit.

You will need to enter your mailing address and it does say while supplies last.  Please note that it took me a few minutes to get the form to work.

I got this confirmation saying my free book would be here in 3-5 weeks.
