My guess is one per household.
I figured that was the case, but I didn’t see it anywhere.
Very bottom of the page, one per household.
Under the form it says “Limit one sample pack per person, household or address. Additional requests may constitute fraud.”
I spent almost 45 minutes earlier trying to get this to work and finally gave up. Took me less than a minute tonight. Thanks for the update!
