I just purchased my 30.00 of products and choose to decline mag and get refund. do I print the form and mark everything on paper or fill the on-line form out AND mail in the paper. Thanks for your help!

I have the same question as Debbie. I’m ready to submit for the $12 rebate but am unclear as to whether or not I need to fill in the online form (clicking “no thanks” for the magazine) as well as print off the form and send it in. Have you heard the correct way to do this?
